When a Foot Problem May Start Higher Up
When your foot or ankle hurts, it is natural to focus on the area that hurts. You may look at your shoes, change your walking habits, or assume the problem is isolated to the lower leg. Sometimes it is. But the body does not move as a collection of separate parts.
Your hips, knees, ankles, and feet work together every time you take a step, climb stairs, run, or change direction. If the muscles around your hip are not providing enough strength or control, other parts of the leg may have to compensate.
That does not mean every foot or ankle problem originates at the hip. It does mean that looking at the entire movement pattern can sometimes reveal something that treating the painful area alone misses.
How Does the Kinetic Chain Affect Your Foot & Ankle?
The lower extremity functions as a kinetic chain. Movement at one joint can influence what happens at the joints above and below it.
The hip plays an especially important role because it helps control the position of the thigh and pelvis as you move. When the muscles around the hip cannot adequately control the movement, the knee, ankle, and foot may respond by altering their mechanics.
Consider what happens during a single-leg movement, such as walking:
- The Hip Controls the Thigh. The muscles surrounding the hip help stabilize the pelvis and control the position of the femur as your body moves forward.
- The Knee Responds to Changes Above It. Altered control at the hip can affect how the knee tracks and how force travels through the leg.
- The Ankle Adapts to the Movement. The ankle has to help maintain balance and absorb forces as the foot contacts the ground.
- The Foot Provides a Foundation. The foot must adapt to the surface while helping transfer force back through the rest of the leg.
A problem in one part of this sequence does not automatically cause a problem elsewhere. However, inefficient movement in one area can increase the demands placed on another.
Can Weak Hips Cause Foot or Ankle Pain?
Hip weakness is not necessarily a direct cause of foot or ankle pain. The relationship is more nuanced.
Weakness or poor motor control around the hip can change how the leg moves during weight-bearing activities. For some people, this can contribute to movement patterns that place additional stress on structures farther down the leg.
You may notice this most during activities that require greater control, such as:
- Running. Repeated single-leg loading places significant demands on the hip and lower extremity.
- Walking Quickly. Increased pace requires the leg to absorb and produce force more efficiently.
- Climbing Stairs. The hip and knee must generate force while the foot and ankle provide a stable base.
- Squatting. Limited hip control can change how force is distributed throughout the lower extremity.
- Cutting or Pivoting. Rapid changes in direction require coordinated control from the hip through the foot.
The important question is not simply whether your hips are "weak." A physical therapist can look at whether your muscles are strong enough and whether they are working at the right time and in the right way.
What Does Hip Weakness Look Like During Movement?
You may not feel weakness in your hip at all. In many cases, it becomes more apparent when you perform a movement that requires the leg to support your body.
Some possible signs include:
- Your Knee Moves Inward. During a squat, step-down, or single-leg movement, the knee may drift toward the middle of the body.
- Your Pelvis Drops. Difficulty stabilizing the pelvis on one leg can create a noticeable shift during walking or running.
- Your Foot Changes Position. Changes higher up the leg can influence how the foot interacts with the ground.
- You Rely More Heavily on One Side. You may unconsciously shift your weight away from a weaker or less stable side.
- You Struggle with Single-Leg Control. Difficulty maintaining alignment during a single-leg task can reveal deficits that are not obvious during two-legged movements.
None of these signs automatically means you have a hip-related cause of foot or ankle pain. They are simply clues that the entire movement pattern may deserve evaluation.
Why Strength Isn't the Only Piece of the Puzzle
It is tempting to assume that stronger muscles automatically mean better movement. Strength matters, but coordination matters too.
Your nervous system constantly adjusts muscle activity based on where your body is in space, the surface beneath you, and what you are trying to accomplish. A person may have adequate hip strength during a controlled exercise but still struggle to use that strength effectively during running or a quick change of direction.
This is where physical therapy for foot and ankle problems can take a broader approach.
A physical therapist may assess:
- Strength. How much force can the hip, knee, ankle, and foot produce?
- Mobility. Do restrictions at one joint affect movement elsewhere?
- Balance. How well do you maintain control when standing or moving on one leg?
- Coordination. How effectively do different muscle groups work together?
- Movement Mechanics. How does your entire lower extremity move during functional activities?
Looking at these factors together can provide a more complete picture than focusing exclusively on the location of pain.
How Can Physical Therapy Address the Whole Leg?
Treatment depends on what the evaluation shows. If hip weakness or poor control contributes to an inefficient movement pattern, rehabilitation may incorporate exercises that address the hip while also targeting the knee, ankle, and foot.
A program may include:
- Hip Strengthening. Targeted exercises can improve the muscles responsible for stabilizing and moving the hip.
- Single-Leg Exercises. These activities challenge strength and coordination while more closely resembling everyday movement.
- Lower-Extremity Control. Exercises can help you practice maintaining alignment as the hip, knee, ankle, and foot work together.
- Balance Training. Improving balance can help the entire leg respond to changes in position and surface.
- Functional Progression. Treatment can gradually move from controlled exercises toward walking, running, recreational activities, or other movements that matter to you.
The objective is not to strengthen your hips simply because hip strength is important. It is to address the factors that may be affecting how your entire lower extremity functions.
Look Beyond the Location of Pain
Your body does not stop working as a connected system simply because one area hurts. A foot or ankle complaint may involve local factors, but movement at the hip and knee can also provide useful information about how forces are traveling through the leg.
